Notes on Health questions:

The questions related to your health are designed to see whether you have any health problems that may affect your ability to undertake the duties of the post you have been offered or place you at any risk in the workplace.

Medication Question - For example medicines that make you drowsy and prohibit you from driving any plant or operating machinery

Health Conditions / Disabilities Question - For example back, joint, eyesight, hearing, high blood pressure problems, chest complaints, mental health condition, diabetes, asthma, etc.

If you have answered yes to any of these Health related questions please discuss with Nathan Forrest directly.
